Position Overview:

The Clinical Manager ensures all services provided by the Agency are sufficiently planned, coordinated and implemented. This position provides evaluation, direction and application of the Plan of Care and treatment according to state licensing laws. This position is responsible for review of OASIS and Plans of Care regarding appropriateness, completeness, and compliance with federal and state regulations and Agency policy.

Experience and Skills

Job Requirements:

  • Associate degree or Bachelor of Science in Nursing OR Master or Doctor of Physical Therapy, Occupational Therapy, Speech Language Pathology/Therapy
  • Licensed professional in state of Illinois
  • Valid Driver’s License required
  • Valid Auto Insurance required
  • Minimum of 2 years as a registered professional in the healthcare field
  • Minimum of 1 year in home care
  • Knowledge of Medicare and Medicaid regulations
  • Ability to communicate effectively across organization; with patients, volunteers, and other customers
  • Ability to function effectively in a fast-paced and changing environment with multiple priorities and objectives; to operate with a sense of urgency while maintaining flexibility and effectively managing priorities meeting deadlines
  • Ability to maintain organizational confidentiality which includes, but in not limited to, information about patients/families, volunteers, employees, donors and Agency processes
  • Excellent communication skills, verbal and written
  • Organizational skills
  • Computer skills, including EMR

Preferred Experience, Skills, and Qualifications:

  • OASIS certification (HCS-O, COS-C)
